ChatGPT recommends the med spas it can verify.
When a patient asks "where should I get natural-looking Botox near me," the model looks for clinics whose treatments, providers, location and reputation are stated clearly and repeated by sources it trusts. Clinics with a beautiful website but little outside proof usually don't make the list.
Most med spas don't exist to AI yet
The gap is bigger than most owners realize. In its Med Spa & Aesthetic Medicine AI Visibility Index 2026, 5W found that the top 25 brands capture roughly 95% of AI citation share in the category. The remaining 11,500 or so independent med spas split about 5%.
Most of that top 25 aren't clinics at all. They're the product and device brands patients ask about by name. So when someone asks about wrinkles, AI talks about the treatment first and the clinic second, if at all.
What AI looks for when it does name a clinic
When the question is local and specific, AI shifts from "what treatment" to "who should I trust." Four things decide whether your clinic makes that shortlist.
- 1. Treatments it can match to the question. If a patient asks about acne scars and your site says "skin rejuvenation," the model has to guess. Clear treatment pages, named the way patients ask, make the match easy.
- 2. Providers it can verify. AI gives weight to credentials. Name your injectors and medical director, state their qualifications, and keep those details identical on your site, Google Business Profile and any directory listings.
- 3. Local facts that agree everywhere. Your name, address, hours and services should match across your website, Google, Yelp, RealSelf and booking platforms. Small inconsistencies make AI less sure it has the right clinic.
- 4. Proof from outside your own site. Reviews, local press, professional associations and directory profiles all count. AI trusts what others say about you more than what you say about yourself.
Why your Google ranking isn't enough
Ranking well in local search and being recommended by AI are different jobs. Search shows ten options and lets the patient compare. AI often gives one answer, sometimes three, and explains why. To be in that answer, your clinic needs to be easy to describe, not just easy to find.
What to do this month
- Rewrite treatment pages around the questions patients actually ask, with the answer in the first two sentences.
- Add a provider page for every injector, with credentials and the treatments they specialize in.
- Audit your listings so your name, address and services match everywhere.
- Ask happy patients to mention the specific treatment in their reviews. "Lip filler with Dr. Lee" tells AI far more than "great experience."

Questions
- Does ChatGPT use Google reviews to recommend med spas?
- It uses whatever sources it can find and trust, and review platforms are among them. Volume matters, but specific reviews that name treatments and providers matter more.
- Can a small med spa compete with chains in AI answers?
- Yes, for local and treatment-specific questions. AI favors the clinic it can verify most clearly, not the biggest one.
- How long does it take to show up in AI answers?
- Fixing your own site and listings can show movement within one to three months. Earning outside mentions takes longer and compounds over time.
